Online Survey

This is the list of questions to help you prepare your responses to the online survey form.
The form is to be completed by leaders ages 16 and above.
If you work across age groups, you may complete the form separately for the ‘younger years age group (5-8)’ and the other age groups. You may also choose the age group most representative of your group.
You will get a link to download the certificate of participation at the end of the online form.
